Non-Healthcare Reporting
1) REPORTING CASES IN EMPLOYEES, PATRONS OR VISITORS:

2) REPORTING CASES AMONG PATIENTS AND RESIDENTS
In accordance with DCMR Chapter 22B 201.1(ff) and 201.1 (gg) and as part of the District of Columbia’s (DC’s) Coronavirus 2019 (COVID-19) response, healthcare providers must report negative and positive test results for all COVID-19 diagnostic and serologic testing, including point-of-care testing. Reports should be submitted electronically using DC Reporting and Surveillance Center (DCRC), our online reporting system.
- Healthcare providers should submit a Notifiable Disease and Condition Case Report Form online using the new reporting platform
- Testing facilities, including laboratories, may report batched negative results using the line list template when prompted in the COVID-19 reporting form
- Healthcare providers should report a death associated with COVID-19
- Laboratories are recommended to submit COVID-19 results via Electronic Lab Reporting. Please email [email protected] to initiate the process
- Hospitals should report cases in patients/residents via weekly line lists to DC Health using the line list template: COVID Reporting Template_Hospital_Inpatient
- Skilled nursing facilities and assisted living facilities should report cases in patients/residents via weekly line lists to DC Health using the line list: COVID Reporting Template_LTCF
3) REPORTING CASES AMONG HEALTHCARE PERSONNEL
In accordance with DCMR Chapter 22B 201.1(ff), 201.1 (gg) and 208.2 and as part of the District of Columbia’s (DC’s) Coronavirus 2019 (COVID-19) response, all healthcare settings are to begin timely reporting of all healthcare personnel infected with COVID-19 who work in the District of Columbia.
All deaths should be reported within 24 hours to [email protected].
Hospitals:
- Report cases among healthcare personnel using the line list template: COVID Reporting Template_Hospital_Employee
- Cases should be reported by 3pm every Wednesday
- Send the updated line list to [email protected]
Nursing homes and assisted living facilities should:
- Report cases among healthcare personnel using the line list template: COVID Reporting Template_LTCF
- Cases should be reported by 3pm every Wednesday
- Send the updated line list to [email protected]
Any outpatient facility that is not a hemodialysis facility (e.g., dental, ambulatory surgical center, cancer clinics, and ambulatory care/primary care) should:
- Report cases among healthcare personnel using the online COVID-19 Reporting Form
- Cases should be reported within 24 hours
