Medical Training License (MTL) New Application
All postgraduate physician residents/fellows practicing in District hospitals and their affiliated academic training institutions, must apply to the DC Board of Medicine for a medical training license (MTL) prior to their program onsite start date. The MTL is a limited license and practice is solely limited to the training program institution listed on the application.
The processing of an application depends on many factors. To avoid delays, please read all requirements and application instructions carefully prior to completing your application.
Your GME office will serve as the liaison between you and the DC Board of Medicine. Due to the large volume of postgraduate physician residents/fellows applying for a medical training license, questions about information being requested on the license application form, and the process, should first be directed to your GME office.

All applicants renewing their licenses must be in good standing with their institution, prior to submitting their application.
The renewal fee is $50. (Please print a copy of your payment confirmation for your records and for your GME office.)
To renew, licensees must utilize the online license application located here. You will need to register before logging in.
Medical Training Registrant (MTR)
Medical residents who are rotating for less than 120 days in any of the academic training institutions in the District must complete the medical training registrant (MTR) form.
Your GME office will serve as the liaison between you and the DC Board of Medicine. Due to the large volume of postgraduate physician residents applying for the MTR, questions about information being requested on the application form, and the process, should first be directed to your GME office.
Please be advised that MTR applications must be submitted no more than 90 days before the start of the rotation. Applications submitted beyond this 90-day timeframe will be closed, and applicants will be required to reapply and pay all necessary fees.
Please note, all new MTL and MTR applicants must undergo a criminal background check (CBC) as part of the application process. In addition, some applicants may be required to meet additional requirements, including meeting with the Board, prior to being issued an MTL or MTR.
All MTL and MTR approvals will be available online and can be found using the online professional license search tool. No hard copy licenses will be mailed.
