Office of Health Facilities Licensing Portal

Please review the following licensing alerts before applying for a license:

Home Care Agency Applicants: On May 13, 2026, the Centers for Medicare and Medicaid Services (CMS) issued a six (6) month moratorium for new enrollment. If you are seeking licensure as a Home Care Agency to qualify for certification as a CMS provider, please take note of this important information, prior to applying. Information regarding the Home Health moratorium can be found in the Federal Register - Home Health.

Hospice Agencies: On May 13, 2026, the Centers for Medicare and Medicaid Services (CMS) issued a six (6) month moratorium for new enrollment. As the State Agency, DC Health will not process initial certification requests for Hospice Agencies to qualify for enrollment as a CMS provider until the moratorium is lifted. Information regarding the Hospice moratorium can be found in the Federal Register - Hospice.

Community Residential Facilities (CRFs) for Individuals with Intellectual Disabilities: In 2018, the Department of Health Care Finance (DHCF) determined that the District had reached capacity for Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID). Accordingly, effective June 1, 2018, DHCF commenced its denial of all new applications submitted by ICF/IIDs for enrollment as a District of Columbia Medicaid provider. Therefore, DC Health will refrain from issuing licenses for Community Residential Facilities (CRFs) for Individuals with Intellectual Disabilities until the moratorium has been lifted. We are pleased to announce that the portal for Healthcare Facilities licensing is now fully functional. The system will allow you to manage your licenses for health facilities with greater ease and efficiency. Current licensees are now able to access their existing licenses and apply for renewals and changes, including Change of Ownership. The portal is also open to apply for a new license for a health facility.

Please be advised that, effective May 3, 2023, the Office of Health Facilities will no longer accept paper applications. Paper applications submitted to the Office of Health Facilities on or before May 3, 2023, will be processed without delay.

All future licensing requests for existing and new providers, as well as payment, will be received through this portal.
