NOTICE

The pharmacy renewal portal is now OPEN.

The renewal is available online ONLY. No paper applications will be accepted. 

The Controlled Substance Registration renewal application is available in the portal as well. Answer "yes" to the queston, "Would you like to renew a Controlled Substance Registration?" in order to renew. You will be prompted to upload the required documents during the renewal process.

Please allow at least 15 business days for application review. If your application requires additional information, a health licensing specialist will contact you. You will receive a copy of the renewed license via email, upon approval.
