The pharmacy renewal portal is now OPEN.

All resident and non-resident pharmacies must submit a renewal application before licenses expire on May 31, 2021.  The renewal is available online ONLY. No paper applications will be accepted. 

The Controlled Substance Registration renewal application is available in the portal as well. Answer "yes" to the queston, "Would you like to renew a Controlled Substance Registration?" in order to renew. You will be prompted to upload the required documents during the renewal process.

The checklists below list all documentation needed for Pharmacy license and Controlled Substance Registration renewal.
