Revoked by Surrender

Papp, Esther (1/8/15) – The physician’s medical training license was administratively revoked after surrendering while the Board was addressing compliance with her previous private order.  [Medical Resident]

Esfahani, Mahin Z. (1/8/15) – The physician’s license was administratively revoked after surrendering while facing disciplinary action by the Board.

Kump, Leila (4/29/15) – The physician’s license was administratively revoked after surrendering while facing disciplinary action by the Board. [Opthalmology]

Denied

Ohiokpehai, Tajudeen I. (6/24/15) – The physician’s application for licensure was denied based on multiple disciplinary actions taken by the Maryland Board regarding malpractice, standard of care, and fraudulent application.

Udebiuwa, Oparaugo I. (10/28/15) - The physician’s application for licensure was denied based on Board’s decision not to terminate the current revocation for multiple disciplinary actions taken by the Maryland and DC Boards. [Psychiatry & Neurology]

Suspended

Yu, Benson Waison (1/8/15) – The physician’s license was suspended for 18 months after vacation of the Summary Suspension of 10/4/13, then is prohibited from prescribing,administering, or dispensing any Schedule II, III, and IV controlled substances for a minimum of 2 years. [Internal Medicine]

Choudry, Mubashar (1/28/15) – The physician’s license was suspended indefinitely, until he can comply DC and Maryland terms.  This action was based on a Maryland action for unprofessional conduct and providing services while under the influence of alcohol.

Diggs, James (3/25/15) The physician’s license was indefinitely suspended until full compliance with delinquent CMEs and fine imposed. This action was based on misrepresentations on renewal applications regarding CMEs and failure to disclose past arrests. [Internal Medicine].

Rassael, Hadi (3/26/15) – The physician’s summary suspension of 11/20/14 was vacated and license was then suspended for 18 months effective 10/7/14.  Upon termination of the suspension, physician is then placed on probation for a minimum of 3 years with terms. This action was based on a Maryland summary suspension for engaging in inappropriate relationships with at least three patients. [Family Medicine]

Malik, Ishtiaq (8/12/15) –  The stay of the physician’s January 9, 2014 suspension is vacated, and the license is now fully suspended for one year. The suspension is based on his violation of the 1/9/14 probation, which had been imposed based on a Maryland action for standard of care issues and fraud, and also a DC violation for allowing the unlicensed practice of an individual in his practice.  [Internal Medicine]

Fined

Chevalier,Denise (1/28/15) – The physician was fined and required to complete delinquent CME based on a Maryland action for failing a CME audit, and also being delinquent for DC.

Andrews, Newton (5/27/15)- The physician was fined based on failure to comply with the District of Columbia’s CME requirements for licensure renewal.

Keats, Aminah (8/27/15)  The physician was fined based on practice without a license.  Also, must complete CMEs and notify all patients seen while unlicensed. [Naturopathy]

Reprimanded

Abbruzzese, Mark (1/8/15) –   The physician was  for failure to properly dose/monitor Coumadin. [Internal Medicine]

Derakhshan, Iraj (3/26/15) – The physician was reprimanded based on a West Virginia Board action regarding unprofessional conduct and HIPAA violations.

Brown, Carl (6/24/15) – The physician was reprimanded based on a Virginia Board action dated 10/1/14 regarding standard of care related to obstetrics and gynecological surgery. [Obstetrics & Gynecology]

Tarantolo, Joseph (11/25/15) – The physician was reprimanded and fined based on failure to meet the standard of care and willful or careless disregard for the health of a patient. [Psychiatry]

Thurston, Candace (11/25/15) – The physician was reprimanded based on a Virginia board action dated 3/30/15 regarding inadequate examination of a patient, improper surgery, fraudulent documentation, and inadequate post -operative care. [Obstatrics & Gynecology]
