Revoked 

Jemsek, Joseph (8/14/25) – The physician’s license was revoked based on multiple standard of care violations related to the treatment of Lyme Disease.

Raley, Thomas (6/20/25) – The physician’s license was revoked based on a felony conviction and three years of incarceration, for prescribing violations and a kickback scheme.

Summarily Suspended
 
Jemsek, Joseph (5/13/25) – The physician was summarily suspended based on concerns regarding impairment.
 
Wills, Reginald  (07/16/25) – The physician was summarily suspended based on prescribing violations.

Kump, Leila (10/21/25) – The physician’s license was summarily suspended based on a Virginia suspension for standard of care and prescribing violations related to cosmetic procedures.

Purdy, Laura (10/28/25) - The physician’s license was suspended and immediately stayed, based on a Mississippi action for telemedicine violations.

Probation

Araya, Henok (8/20/25) – The Physician was placed on probation with restrictions and terms, based on multiple complaints regarding standard of care, misrepresentation, disregard for health and welfare, and fraud-- all relating to liposuction procedures.

Fined

Potthoff, Troy (5/5/25) – The Physician was fined and reprimanded based on a Virginia action for telehealth violations.

Majete, Lisa (2/18/25) –  The Physician was fined and reprimanded based on a Virginia action for failure to supervise medical assistants in her practice.

Adelakun, Adeniyi (10/28/25) – The Physician was fined based on a Delaware action for failure to complete required CMEs, and failure to complete DC’s as well.

Modi, Navita (11/25/25) – The Physician was reprimanded based on a Maryland action for standard of care issues and inadequate record keeping.

Mueller, Dana (5/6/25) – The Physician was reprimanded based on a Maryland action for allowing the practice of an unlicensed person.
