Garada, Hazem (10/31/23) – The physician’s application for licensure was denied, based on prior disciplinary actions, including suspension and revocation, for felony healthcare fraud.  
 

Probation 

Nedd, Wilton O. (5/22/23) – The physician’s license was placed on probation and reprimanded based on a Maryland action regarding amputation of a foot and failure to maintain records. 
 

Reprimanded

Dadgar, Shabnam (2/7/23) – The Physician was reprimanded and restricted from cryosurgery, based on a Maryland action for failure to meet the standard of care and inadequate record keeping. 
 

Summarily Suspended

Cruz, Andrew (12/20/23) - The physician’s license was summarily suspended based on Arizona and North Carolina actions regrding substance abuse treatment. 

Bentley, Keri M. (5/8/23) - The physician’s license was summarily suspended based on a Kentucky action for a felony conviction regarding controlled substance violations.

Gooding, Frederick (4/20/23) - The physician’s license was summarily suspended based on a felony conviction for 11 counts of Medicaid fraud. 

Raley, Thomas (2/10/23) - The physician’s license was summarily suspended based on a Virginia action regarding two felony convictions for conspiracy to receive health care kickbacks and conspiracy to commit wire fraud.   
 

Suspended

Dormu, Jeffery (2/7/23) – The physician’s 9/12/22 summary suspension is terminated, and license is suspended and reprimanded, then to be placed on probation, based on a Maryland action regarding unnecessary invasive procedures, incorrect diagnosis, and inadequate records. 

Kumar, Asheesh (5/2/23) - The physician’s 8/11/22 summary suspension was lifted, and replaced with a reprimand, suspension, and probation with terms, based on a Maryland action regarding intoxication while working and employment termination.

Riyaz, Farhaad (8/21/23) – The physician’s 4/14/22 summary suspension was terminated, and license was suspended, later to be placed on probation, based on a Colorado suspension regarding a felony conviction for wire fraud and moral turpitude.
