Variance Request Guidance Document

There may be an occasion when a Food Establishment will need to submit a variance request along with a HACCP Plan submission.  Please be sure to check with the Food Technologist if you are to submit a Variance Request.  

If you are submitting a variance request, please read the instructions carefully, as well as be sure to include the complete, total fee.
