The Asthma Action Plan (AAP) is a useful tool to help school staff, parents, guardians, or anyone looking after your child understand their asthma symptoms, asthma triggers, and medication dosage.

This form should be filled out by your child's primary care provider once a year and given to your student's schools front office as well as any adult who watches over them.
