Privacy Statement

DC Health created this Virtual Assistant system for use by individuals or their parent/guardian/legal representative who wishes to update or correct their DC Health COVID-19 Digital Vaccine Record. The information is collected and will be kept confidential and on file as required by law.

The following data fields are mandatory: first and last name, mobile number, date of birth, address (including street, city, state, ZIP Code), vaccine brand, number of doses, date of vaccine(s), name of clinic at which the vaccine was received, and an email address. In some situations, you may also need to upload a copy of the identification card or proof of identity documents for the individual requesting a correction. Failure to provide the mandatory information will result in an inability to use the Virtual Assistant to update your COVID-19 Digital Vaccine Record.

The remaining data fields are optional, however, providing the optional data may help locate your information.

Note that any information collected by the Virtual Assistant may be disclosed to other state and federal agencies, as required by law. You have the right to review records DC Health maintains about you. DC Health will, upon request, inform you of the location of your records and the categories of persons who use the information in those records. For more information, contact us at: 1-855-363-0333.
