Walls-Kaufman, David (12/01/17):  This chiropractor was fined $900 for practicing chiropractic without a license.

Yousefi, Anahita (01/01/2014): This chiropractor was fined $300 for failing to comply with the CE requirements to maintain license.

 

2013

Zahra Yousefi (8/14/2013): This chiropractor was fined $200 for failing to comply with the CE requirement to maintain licensure. This chiropractor was also ordered to complete the deficient CE credits along with CE requirements for the new renewal period.

Reza Zandieh (8/8/2013): This chiropractor was fined $500 for failing to answer the Board’s inquiry and an additional $1,200 for failing to comply with the CE requirement to maintain licensure.
