The Pharmaceutical Control Division is extending the renewal period to Wednesday, June 30, 2021. This means that your resident or non-resident pharmacy license and controlled substance registration do not expire until June 30, 2021. All renewal applications must be submitted by May 31, 2021 to avoid late fees.

The renewal is available online ONLY. No paper applications will be accepted.
The Controlled Substance Registration renewal application is available in the portal as well. Answer "yes" to the queston, "Would you like to renew a Controlled Substance Registration?" in order to renew. You will be prompted to upload the required documents during the renewal process.
The checklists below list all documentation needed for Pharmacy license and Controlled Substance Registration renewal.
Please allow at least 15 business days for application review. If your application requires additional information, a health licensing specialist will contact you. You will receive a copy of the renewed license via email, upon approval.
