Unity Health Care to Transfer Patients from Woodridge Health Center in NE

DOH is investing in expanded healthcare options for Ward 5.

Washington, DC – The District of Columbia Department of Health (DOH) and Unity Health Care announced that on June 30, 2010, the Woodridge Health Center, located at 2146 24th Pl NE will be permanently closed, and all patients will be transferred to other available sources of care.

Unity Clinic currently operates the Woodridge facility and six other locations with the help of DOH funds, and is responsible for nearly 3,000 patients at the Woodridge location. Unity is sending notifications of the closure to patients, as well as information on patient transfer efforts.

Unity will reach out to patients to notify them of the upcoming change with letters, telephone calls, during patient visits, and through the Patient Scheduling Center. Patients will have the option to remain within the Unity healthcare system or be transferred to non-Unity health center sites. Unity’s recent adoption of an organization-wide electronic health record (EHR) will ensure continuity of care for all patients remaining in the Unity network. Patients who choose to transition their care to a non-Unity provider, will have their records transferred within five business days after Unity has received the signed patient medical records release authorization.

DOH will continue to fund and support Unity Health Care and its six other locations, and will continue to help Unity accommodate displaced Woodridge patients and staff. Recognizing the need for additional health care sites in Ward 5, DOH has invested a total of $3.1 million in funds towards the development of a new Unity site in the Brentwood area and has invested in renovation and expansion efforts of the So Others Might Eat (SOME) center.
