Mission
The mission of the CHP is to build unified and effective approaches to preventing, controlling and limiting the morbidity and mortality associated with cardiovascular diseases in the District of Columbia.
The focus of the CHP is centered upon seven listed goals:
1. Develop and coordinate partnerships 2. Develop scientific capacity to define the CVD problem 3. Develop an inventory of Policy and Environmental strategies 4. Develop a 5 year State Plan 5. Provide training and technical assistance 6. Develop population-based strategies 7. Develop culturally-competent strategies for priority populations
Program Description
The Cardiovascular Health Program (CHP) is located in the DC Department of Health, Primary Care and Prevention Administration, Bureau of Primary Health Care. It is funded through a grant with the Centers for Disease Control and Prevention (CDC).
The CHP is involved in several activities and has several planned for the future. Currently, the CHP is formulating a District-wide assessment of cardiovascular disease risk and perception among District residents. This assessment will include a sampling of District worksites, schools, managed care organizations and communities within each ward. The CHP plans to utilize the information gained to solicit additional funding and plan for future initiatives.
Several partnerships have been made with other DOH programs including Comprehensive Cancer Control Planning & Fitness Initiative, Diabetes and Tobacco Control and BRFSS. Other contributing organizations include, but are not limited to, the American Heart Association, the Network to Improve Community Health, Delmarva Foundation, DC Medical Society, Howard University Hospital, and YMCA.
These partnerships and others have helped to create the Cardiovascular Forum. This Forum has brought a broad group of community stakeholders together to plan and implement a regional cardiovascular initiative. Although the final products and activities will be decided by the Forum participants, we anticipate that a set of tasks will be undertaken: a coordinated regional initiative to prevent and reduce cardiovascular disease, including healthcare provider assistance, patient assistance, and community awareness.
