The following form is to be used by licensed health care practitioners to report communicable diseases other than HIV/AIDS, STDs, Tuberculosis, and Hepatitis to the District of Columbia Department of Health. Please print the Disease Report Form, fill-in the information and fax the completed form to the Division of Disease Surveillance and Investigation at (202) 442-8060. To report HIV/AIDS or Hepatitis, contact the HIV/AIDS Administration at (202) 671-4900.

To report STDs, contact the STD Control Program at (202) 727-9860. 

To report Tuberculosis, contact the Bureau of Tuberculosis Control at (202) 698-4030.
